For a small business quote

  • Number of employees and their home ZIP codes
  • Your current plan and latest renewal, if you have one
  • Roughly what you can contribute per employee per month
  • Whether any employees have coverage elsewhere, through a spouse or Medicare

For Medicare supplement help

  • Your date of birth, or your Part A and Part B effective dates
  • Your ZIP code
  • Any coverage you have now and would be replacing
  • A list of your regular prescriptions
